Peri-orbital Rejuvenation

Question: Ever since I had a facelift, my friends say I look more tired. I’ve noticed that the aging around my eye area is more pronounced since I had surgery. What are my options for rejuvenating my upper face?

Answer: Whenever a rejuvenation procedure is recommended, it is important that your surgeon take into consideration your overall facial balance. When the upper face and/or eye area is aging, rejuvenation of the lower face/neck area can bring more attention to an aging upper face if left untreated.

Heaviness of the upper eyelids can be improved with an eyelid lift where a very fine incision is hidden in the upper eyelid crease to remove excess skin and bulging fat. Heaviness of the lower eyelid can be treated at the same time through a hidden incision just under the lash line although often an external incision is not even needed for the lower lid area. I perform 95% of my upper and lower eyelid lifts under local anesthesia in the office without the need for IV sedation.

Wrinkles between the eyes, above the brows and crows feet are most effectively treated with a neuromodulator. Botox cosmetic, Dysport and Jeuveau are equally effective in my hands. Wrinkles under the eyes can also be softened with neuromodulator.

Many patients underestimate the value of the temple in rejuvenation of the upper face. A hollow temple makes the eyes appear more narrow and less rested. Volumization of the temple can be achieved with temporary one year fillers such as Restylane, RHA collection or Juvederm. Bellafill – the 5-year filler – is the most commonly requested filler in my office for both temple hollows as well as for the appearance of dark circles under the eyes which can make one appear tired. Bellafill is popular due its longevity and natural results.

Both non-surgical and surgical options are available for rejuvenation of the upper face. A detailed consultation by a Board Certified Facial Plastic Surgeon experienced in the full spectrum of surgical and minimal invasive facial rejuvenation techniques is the first step. Superior technical skill, sound judgement and an artistic eye are equally important considerations to maintain a natural balance and proportion to the face.
